The news story follows, it is the fourth crash of the Mk208 and the first to be non-fatal. Based on the little information, it was most probably an engine failure.
For the other three Malaysian Defence could only make educated guesses as RMAF have never released the investigation report.

KUALA LUMPUR, May 4 (Bernama) — A Royal Malaysian Air Force (RMAF) Hawk 208 plane crashed after take-off at 10.32am at the Kuantan Airbase today.
However, the pilot ejected to safety and did not suffer serious injuries, RMAF public relations officer Major Zulkiflee Abdul Latiff said in a statement.
The pilot has been brought to the Tengku Ampuan Afzan Hospital in Kuantan.
Zulkiflee said the aircraft, which is based at the airbase, was on a routine flight training when the mishap happened.
“It crashed within the airbase soon after take off,” he added.
He said a board of inquiry would be set up to investigate the incident.
